I need a member to help fill my forms in ukraine language

Hi members, I am 31 years old born in Lvov-ukraine to a nigerian father and a ukrainian mother , i have lived in africa for so many years now and do not know how to speak ukrainian language. the situation is that i recently visited the Ukrainian embassy in order to start the process of renewing my old passport and i was given forms to fill in , the writings where however in ukrainian language.
I would like to scan and send the same forms to any member who will help to translate it to english and send back to me , after i fill the forms i send it back again so that it can be translated back to ukrainian language before i then go ahead to submit the filled forms at the embassy.
