Church in Borki

I am trying to obtain my father's Christening certificate, but do not know of the name of any Greek Catholic churches in Borki. This town is somewhere outside of Lviv. Does anyone know what this church would be?

There are 3 Borki villages in Lviv Province nowadays:

1. 50° 28′ 15″ N, 24° 25′ 30″ E in Sokal region
2. 49° 54′ 48″ N, 23° 55′ 4″ E in Jaworow region
3. 50° 6′ 33″ N, 23° 44′ 45″ in Zhowkwa region

#3 is quite close to the city - about 60km
Still, #2 is only about 12. It's located on the main European Watershed in the beautiful area of Roztocze:

The first written record on the village (#2) dates back to 1458. This village used to be called Borki Dominikanskie until 1947. The nowadays population is about 2200. Borki Dominikanskie used to belong to Rokytne Greek Catholic parish
There are anly about 250 people living in Rokytne nowadays:
