Hochwatt/Hochman/Hoffman in Ukraine

Hello everyone!

Most of my family is originally from Ukraine. I am lucky, and most of my family was already in the United States by 1923. Some of them escaped Ukraine hiding in a horse-drawn carriage! Here is some information about my family:

My Mother's Side (Hochman/Hofman/Hoffman)

Grandfather: Owsej Hofman (Selvin Hoffman in America), born in Czernakov 1910
Great Grandfather: Efraim Hofman, born in Rudnya 1878, died in Zhitomir during WWI 1916
Great Grandmother: Genia Hochwatt (Hannah Hoffman), born in 1878
Great Great Grandfather: Chaim Hochwatt (Charles Hoffman), born in Radomysl 1853

Now, I do have some questions:

1. On a ship to the US, my great grandfather Abraham Bergmann (Berkman), he listed Walacyzn, Russia as his home. Where is this?

2. My great grandfather Itzhak Fleishmann (Jacob Fleisher) listed Devitkgrod, Russia as his borthplace. Where is that?

One last thing. My mother's father escaped from Ukraine and found their way to Hamburg, taking a ship to the United States. Here is a copy of their travel paper. Has anyone seen anything like this before?
