I don't know if it was in the news at all, I heard this from the people who work on mine safety issues:
1. The mine management sent the second shift of miners down without waiting for the first one to get on the ground. Allegedly, at that time it was already known the fire has started. So they were sending people INTO THE FIRE
2. When the people (despite all) escaped from the fire to another hall, the management directed the exhaust air collected by the ventilation from the area on fire into the area where the miners were at the moment. Because of the fire, the air contained CO and other combustion products.
3. People did not have masks.
Everyone suffocated.
